Learning Objectives
At the conclusion of this activity, participants will be able to:

| • | Establish a plan to implement improvements in their office-wide systems for the case management of their patients with type 2 diabetes | |
| • | Outline a systematic approach in their practice to the diagnosis and treatment of type 2 diabetes that leads to improved long-term outcomes |
Program Description
This 1-hour Webcast will provide primary-care clinicians with evidence-based strategies to facilitate the optimal management of patients with type 2 diabetes. This program will include two 15- minute didactic presentations followed by a case study.
Agenda
Type 2 Diabetes: Meeting Our Management Goals
This presentation will use a case example to illustrate best practices in the management of type 2 diabetes patients with A1C levels around 7.5%. Appropriate management goals will be highlighted, and regimens utilizing combination oral agents will be discussed.
When and How To Intensify Therapy
A case study will be used to show evidence-based strategies in the management of the type 2 diabetes patient with A1C levels that remain uncontrolled, despite the use of combination oral medications. Intensification of therapy and initiation of insulin will be discussed.
Case Study
Clinical experts will share practical tips and strategies for improving the management of patients with type 2 diabetes in primary care.
All presentations will incorporate data from an ongoing primary-care Performance Improvement program on type 2 diabetes. Available at http://www.mentorqi.com.

OBTAINING CME/CE CREDITS
Upon completion of your participation in the live or archived program, participants will be asked to complete a post-test and evaluation form. Successful completion of the post-test is required to earn Category 1 CME credit. Successful completion is defined as a cumulative score of at least 70% correct. Upon successful completion of the post-test, a certificate of completion will be issued to you.
